Students who wish to enrol at Five Islands Secondary College as part of the Big Picture Education program can do so by downloading and filling in this form:

Once the form has been filled in, either hand it in to our office in person (open hours are 8am-4pm, Monday-Thursday), or email a scanned copy to our email address:
